How to Setup Your Patient Portal Account

Click on “Create an Account”

Complete the online form

Click “I Accept” after reading the terms

Create a new user name and password

Under the “My Info” tab be sure to add an email and/or cell phone number and set your preferences to receive alerts for appointments and updates to your medical record
